Dear Ann:

I am a CNM who has been informed by the hospital corporation that employs me that it is changing malpractice insurance companies (it was previously self-insured). As of December this year, the corporation will no longer allow us to deliver in the 13-year old OOH birth center we’ve owned outside our employment by the medical corporation.

Because the midwives’ contracts include allowance for OOH birth 2015 (though our medical director’s only extends until October of this year), we are considering finding an attorney who can advise us as to our options. But, according to the birth center’s medical director, if our practice is willing to purchase our own OOH insurance, we will still be eligible to do deliveries in the birth center. We don’t know yet if this is actually true.

Could you help us out with a ballpark figure as to the cost of annual coverage? This so that we don’t have to figure out the numbers required by your application before August 29, when the medical director will be meeting with the hospital system’s COO.

Our ballpark figure is 100 births monthly, about 2/3 done by CNMs (employed by the hospital group) and the other 1/3 by privately practicing LMs credentialed to deliver at the birth center. Our birth center is in Tacoma, Washington (The Birthing Inn). The hospital corporation in question is CHI, and the medical group that directly employs us is Franciscan Medical Group.

Thank you for any help you can give us.

Kay Jackson, CNM, ARNP
